Overview

The PCN DES Contractual Reporting dashboards in OneAnalytics provide PCNs and practices with clear, up-to-date views of their performance against national contract requirements. They allow you to:

  • Monitor delivery of key contractual indicators

  • Identify gaps and prioritise action

  • Track progress over time

  • Drill down from PCN-level to individual practices




1. Investment and Impact Fund (IIF)

Purpose:

To track PCN and practice performance against all IIF indicators, ensuring delivery against national targets and maximising achievement.

Key Features:

  • PCN-wide performance overview with indicator-level breakdown

  • Drilldown to practice-level performance for each indicator

  • Month-by-month trend tracking

  • Highlighting of indicators closest to target for priority action

2. Structured Medication Reviews (SMR)

Purpose:

To monitor completion of Structured Medication Reviews, ensuring contract delivery for target patient groups.

Key Features:

  • PCN and practice-level SMR completion rates

  • Breakdown by priority cohorts (e.g. frailty, polypharmacy)

  • Trend view over the contract year

  • Identification of under-served patient groups

3. Cardiovascular Risk

Purpose:

To identify and manage patients at elevated cardiovascular risk, enabling targeted interventions to reduce long-term disease burden.

  • Trend tracking to monitor impact of interventions

4. Core20Plus5

Purpose:

To help PCNs reduce health inequalities by focusing on the most deprived populations and five high-priority clinical areas.

Key Features:

  • PCN and practice-level Core20 (deprivation) counts

  • Plus 5 condition counts: Cancer, COPD, pregnancy in BAME or deprived, hypertension, SMI

  • Area Totals and Plus 5 Totals Trend tabs for time-series analysis

  • Drilldown for identifying specific population needs

5. Hypertension Risk

Purpose:

To highlight patients with elevated blood pressure readings, supporting early intervention and improved control rates.

  • Differentiation between single high reading and multiple high readings

  • Visual prioritisation of highest risk patients (>180/110 multiple readings)

  • Data to support proactive recalls and clinical reviews

Purpose:

To monitor appointment access, capacity, and demand trends using GPAD data.

  • Appointments in 2 Weeks: Percentage available within 2 weeks, with PCN and practice breakdown

  • PCN Appointment Overview: Total appointments and booking-to-appointment time breakdown

  • Appointments Per 1000 Patients: Comparison to national average, and practice vs. PCN average

  • Monthly trend charts to track changes in capacity and demand
