The Structured Medication Review (SMR) dashboard helps PCNs and practices track delivery of SMRs across different patient categories, monitor progress over time, and drill into practice-level performance.
It supports:
Monitoring SMR completion rates.
Identifying patient cohorts where more reviews are needed.
Comparing performance between practices.
The SMR Overview provides a high-level summary for the entire PCN.
Lists each SMR indicator (e.g. SMR01A, SMR01B, SMR01C, etc.).
Shows the number of patients still requiring an SMR in that category.
Line chart showing the number of achieved patients over time for each indicator.
Each coloured line represents a different SMR category.
Helps track whether delivery is increasing steadily.
Switch to Practice Performance view to see individual practice results for each SMR indicator.
Each row represents a practice and SMR indicator.
Bar chart shows:
Green = patients achieved.
Red = patients still to complete.
Patient numbers shown as achieved / eligible.
When you click on an indicator, details are shown including:
Name and definition of the indicator.
Number of patients achieved and eligible.
Shows percentage achievement over time for the selected practice and indicator.
Useful for monitoring improvements after targeted interventions.
Prioritise high remaining patient counts – these represent the biggest opportunity for improvement.
Use PCN overview to spot trends – identify if certain indicators are improving slower than others.
Drill into practice view – to see which sites are driving performance and which may need extra support.
