Hypertension Risk Dashboard
The Hypertension Risk dashboard provides a clear overview of blood pressure control across all practices within your PCN. It helps identify patients whose readings indicate different levels of hypertension risk, enabling targeted intervention and follow-up.
What the Dashboard Shows
Practice-by-practice data within the selected PCN.
Readings categorised by thresholds:

<150/90 mmHg – Controlled BP

<160/100 mmHg – Borderline high BP
-
180/110 mmHg – Significantly high BP
-
>180/110 mmHg – Critically high BP

One Reading – Patients with a single reading in the threshold.

Multiple Readings – Patients with repeated readings in the threshold, indicating persistent concern.
How to Use This Dashboard

Identify at-risk patients – Prioritise those with multiple readings in the higher categories (>160/100 and >180/110).

Plan follow-up – Engage these patients for urgent clinical review, medication optimisation, or lifestyle interventions.

Allocate resources – Identify practices with higher numbers of uncontrolled hypertension to focus resources where they can have the most impact.

Track change – Use periodic extracts to measure improvements or deterioration in BP control at both practice and PCN level.
