Description

HI03

Percentage of patients on the QOF Learning Disability register aged 14 or over, who received an annual Learning Disability Health Check and have a completed Health Action Plan, in addition to a recording of ethnicity.

What makes a patient eligible?

Patients are included in HI03 if they are on the Learning Disability register and are aged 14 or over

How to achieve the indicator

To meet the criteria for HI03, eligible patients must have:
  1. An LD healthcheck completed in the contract year
  2. A LD health action plan completed in the contract year
  3. Ethnicity recorded (ever)

What will remove patients from eligibility?

Patients are can be excluded from HI03 if either:
  1. they chose not to have an LD health check in the contract year, or;
  2. the chose not to have an LD health action plan in the contract year
If a patient has all elements of HI03 recorded, any exclusions that have also been added are ignored.
