Overview
The OneTemplate Dietitian is designed to provide a comprehensive template for Dietitians working in practices and PCNs.
It incorporates a number of elements of OneTemplate Additional Roles including the Introduction page, the Everything page (with a range of useful additional codes) and Common Tools.
Accessing the template
The template can currently be accessed
via the Add template buttons. The template will be made available in the OneLaunchers in the future.
Page Structure
Referral Triage
The Referral Triage page is designed for triaging incoming referrals or assessing requests from colleagues to review patients. Consistent coding of incoming referrals and referral outcomes can help with ensuring this work is picked up on reports.
Assessment
The Assessment page provides sections for history taking and assessment of current diet and activity levels. The full PAL and GPPAQ questions and calculations are on the Common Tools page so this page just has a space to record and comment on the results of those tools.
Examination
The Examination page has standard examination elements which may apply to dietitian consultations. Additional elements may be found on the Common Tools page and those support patients with frailty may also find the Falls/Frailty page useful.
Management Plan and Outcome
The Management Plan and Outcome page includes sections to record an overall management plan, any goals that have been set and information provided to the patient. It may also be used to record if goals so far have been met or not in follow-up appointments. A code of follow-up or discharge then allows for reporting on workload especially where patients are seen for several appointments and it is important to record the overall input per patient rather than just a number of patients seen in a month or quarter.
