Overview

This template has been created to be used by your additional roles team members to record activity and information for patients. The template is based on the specification for the Additional Roles Reimbursement Scheme (ARRS) and it supports the clinician/user in following key clinical, technical and administrative responsibilities in delivering health services. There are different pages on the template for all of the different additional roles, as well as pages for various other clinical aspects to cover responsibilities defined in the specification.

The user would open the OneTemplate Additional Roles and go to the specific page depending on their role. There are additional pages for Care Homes, People Involved, Carer details, and Social Information. As well as an 'Everything' page if you have information to complete for multiple ARRS areas and don't want to go into each page in the template.

The template supports the development of personalised care support plans (PCSP) by holistically bringing together all of the patient's identified care and support needs via information recorded in the template.
If you would like to create a PCSP for a care home, it is advised to use the 'Everything' page to complete the template. This will merge as much information as possible into the care plan.
"Aim for the personalised care and support plan to be developed and agreed with each new patient within seven working days of admission to the home and within seven working days of readmission following a hospital episode" 

The 'Care Homes' page on this template supports Enhanced Health in Care Homes as well as providing digital technology to support the weekly care home round and facilitating the medical input. See the screenshot below which displays part of the page. The Care Homes page also includes the PCSP, structured medication reviews, observations, and other aspects set out in the contract specification. To streamline data entry, the user can record all of the details regarding the weekly care home visit using this page on the template.



There is a 'Pharm Reconciliation' page on the template to support medicines reconciliation, allowing the user to select/code the type of reconciliation, as well as enter other specific medication details (see screenshot below). A full medication review can also be completed using the 'Pharm Review' page.
