033 SMI Physical Health Check DES

Introduction

These suite of searches is written to assist Practices with the SMI Physical Health Check DES. PCNs who have a Local Enhanced Service (LES) for SMI Health Checks should instead referred to their own Service scheme.

Built in line with the Directed Enhanced Service (DES) criteria, searches are grouped into the eligibility cohort to assist in prioritising groups of patients to invite, and then work-to-do to enable the process of achievement. Searches monitor uptake across each individual requirement have been written too and an achievement folder offers different ways of looking at the final figures. 

0) Cohorts



Every patient you can invite is included in the Cohort search. This includes your Psychosis, Schizophrenia, and Bipolar disorder registers. This excludes patients on those registers who have been coded with remissions.
You can use the Population Excluded tab here to identify patients that are in remission.
It's important to note this includes patients on those registers who were diagnosed in the last 3 months, or who were recently registered with the practice in the last 3 months; and that if a patient Declines or does not respond to invites, this does not remove them from the cohort - unlike QOF

This cohort only includes those patients who have had a diagnosis, regardless of if they are on anti-psychotics or mood stabilisers. If they are just on anti-psychotics or mood stabilisers with no diagnosis, they are not included.

1) Work to do


This folder has a selection of searches and reports in which you can identify work outstanding. The top-level reports can be used as an overview, and the searches beneath used as a breakdown.

We provide screening searches here to break down the additional 3 screening programmes.
All elements must be completed within the current Financial Year. If you want to retroactively look back on last year's data, use a Relative Run date on the folder of 31st March.

2) Individual elements Achieved


A breakdown of the core and additional elements are provided so you can easily identify elements that are falling away.
  1. CORE BMI
  2. CORE BP and Pulse
  3. CORE Lipid test
  4. CORE Glucose test
  5. CORE Alcohol consumption
  6. CORE Smoking status
  7. ADDITIONAL Diet and Physical activity
  8. ADDITIONAL Misuse of illicit substances
  9. ADDITIONAL Medicines review
  10. ADDITIONAL All screening programmes up to date
It's important to note that if a patient Declines an individual element, this does not achieve the individual elements. 

3) Achievement



"Core elements of SMI physical health check completed" contains the 6 core elements.

The additional searches:
  1. Additional elements of SMI physical health check completed (excluding screening)
  2. Additional elements of SMI physical health check completed (including screening)
are provided to help you see overall how you are doing with completing all elements for patients (separating out those with screening outstanding to make the impact of this performance easier to understand).

Coding

The OneTemplate Prescriber & Nonprescriber has two pages which cover core+additional elements:
  1. Mental Health page - contains all core+additional elements apart from Blood Pressure
  2. Blood pressure page - BP is on a different page because it would otherwise be repeated on every page on the OneTemplate and is the most frequently used page.
