SMOK002 - Smoking status recorded for long term conditions

Description

SMOK002

The percentage of patients with any or any combination of the following conditions: CHD, PAD, stroke or TIA, hypertension, diabetes, COPD, CKD, asthma, schizophrenia, bipolar affective disorder or other psychoses whose notes record smoking status in the preceding 12 months.

What do the business rules mean?

What makes a patient eligible?

Patients are included in SMOK002 if they have any of the following diagnoses:
  1. CHD
  2. PAD 
  3. Stroke or TIA
  4. unresolved hypertension
  5. unresolved diabetes (in 17 and over)
  6. unresolved COPD
  7. unresolved asthma on treatment (in 20 and over)
  8. unresolved CKD (in 18 and over)
  9. history of psychosis, schizophrenia or bipolar affective disorder

How to achieve the indicator

To meet the criteria for SMOK002, eligible patients must have a current smoker code added within the contract year, and satisfy one of the Group criteria below:

Group 1:
  1. at least 25 years old at the end of the contract year
  2. nost recent smoking status is "never smoked"
  3. "Never smoked" status was recorded on or after their diagnosis date AND this was recorded after their 25th birthday
Group 2:
  1. age under 25 years old at the end of the contract year
  2. most recent smoking status is "never smoked"
  3. "Never smoked" status was recorded within the contract year
Group 3:
  1. latest smoking habit code was "Ex smoker"
  2. "Ex smoker" was recorded in the contract year
Group 4:
  1. Patient has had 3 consecutive years of being coded as "Ex smoker" and no record of being a "current smoker"
  2. Patient has had 3 consecutive years coded as "Ex smoker" which occurred after their latest "current smoker" status

What will remove patients from eligibility?

  1. Patients for whom smoking quality care indicator was unsuitable in the contract year
  2. Patients who declined to give smoking status during the contract year
  3. Patients who chose not to receive the smoking quality care indicator in the contract year
  4. Patients who have been invited twice for review (at least 7 days apart)
  5. Patients who were diagnosed with their first eligible condition in the last 3 months of the contract year
  6. Patient who were newly registered with the practice in the last 3 months of the contract year

If a patient achieves the indicator at any point during the year, any exclusions that have also been added are ignored.
