Indicator ID

Description

SMOK005

The percentage of patients with any or any combination of the following conditions: CHD, PAD, stroke or TIA, hypertension, diabetes, COPD, CKD, asthma, schizophrenia, bipolar affective disorder or other psychoses who are recorded as current smokers who have a record of an offer of support and treatment within the preceding 12 months.

What makes a patient eligible?

Patients are eligible for the SMOK005 denominator if:
  1. their current smoking status is a smoker

How to achieve the indicator

To meet the criteria for SMOK005, eligible patients must have had a recent referral for "Support and refer Stop Smoking Service/Advisor"code for pharmacotherapy, or been issued a drug used in smoking cessation issued in the contract year.

What will remove patients from eligibility?

Patients are can be excluded from SMOK005 if:
  1. Patients for whom smoking quality care indicator was unsuitable in the contract year
  2. Patients who chose not to receive the smoking quality care indicator in the contract year
  3. Patients who have been invited twice for review (at least 7 days apart)
  4. Patients who were diagnosed with the eligible condition in the last 3 months of the contract year
  5. Patients who were newly registered with the practice in the last 3 months of the contract year

Notes
If a patient achieves the indicator at any point during the year, any exclusions that have also been added are ignored.
