This template is for recording all acute conditions. This template covers Asthma Exacerbation, Back Pain, Biopsychosocial Assessment, Chest Pain, COPD Exacerbation, Cough, Depression, Diarrhoea & Vomiting, Ear Ache, Febrile Child, Indigestion, Red Eyes, Sore Throat, Tired all the Time and UTI. 

 

To use this template: 

1. Click on the Acute Conditions button on the PCIT Toolbar

2. From the Cover Page click on the required page  


3. Complete the relevant fields within the template page.  
4. Each page of the template has the usual useful right hand side link, also each template page has buttons which link out specialist page for that particular topic, to as well as links out the NICE/CKS Guidance.
