AF006 | The percentage of patients with atrial fibrillation in whom stroke risk has been assessed using the CHA2DS2-VASc score risk stratification scoring system in the preceding 12 months (excluding those patients with a previous CHADS2 or CHA2DS2-VASc score of 2 or more)

What do the business rules mean?
What makes a patient eligible?
Patients are eligible for the AF006 denominator if:
they have had a CHA2DS2-VASc or CHADS2 stroke risk assessment scoring 2 or more in the contract year, without one previously that is 2 or more.
How to achieve the indicator
To meet the criteria for AF006, eligible patients must have undergone a CHA2DS2-VASc stroke risk assessment within the 12 months leading up to the end of the current contract year.
What will remove patients from eligibility?
- Have had a CHADS2 or CHA2DS2-VASc stroke risk assessment scoring 2 or more at least 12 months before the end of this contract year
- Patients for whom AF quality indicator care was unsuitable in the contract year
- Patients who chose not to receive AF quality indicator care within the contract year
- Patients who did not respond to at least two invitations for review spaced at least 7 days apart within the contract year
- Patients who were diagnosed with AF in the 3 months leading up to the end of the contract year
- Patients who were newly registered at the practice in the 3 months leading up to the end of the contract year

If a patient achieves the
indicator at any point during the year, any exclusions that have also been
added are ignored.
