This template is not linked to QOF or any incentive scheme directly, but it plays an important role in:
Ensuring a complete and accurate clinical record
Supporting practices who wish to manually enter or tidy blood test results
Allowing retrospective entry of paper or secondary care results into structured fields
There are no formal NHS specifications for this template. It simply offers a structured method for manual entry of blood test results into the patient record when results are not received electronically.
Common use cases include:
Adding hospital results that were not filed via pathology links
Recording results shared verbally or on paper (e.g. out-of-hours, private provider)
Documenting bloods taken elsewhere
The template presents a long list of standard blood tests, each with free-text or numeric entry boxes.
You can record values for:
Haematology (e.g. FBC, WBC, platelets)
Renal function (e.g. eGFR, creatinine, urea)
Liver function (e.g. ALT, ALP, bilirubin)
Bone profile
Lipid profile
HbA1c
Thyroid function
Inflammatory markers
Vitamin and mineral levels (e.g. B12, folate, vitamin D)
And many more...
There are no required fields โ simply record the results you need.
Each result is stored as a structured data entry, using SNOMED codes where applicable, enabling searches and recalls to pick them up.
Use this when adding results, such as those from:
Hospital clinics
Private health checks
GP federation services
Walk-in centres or urgent care
These entries will count toward QOF and other KPIs if coded correctly (e.g. HbA1c or eGFR values)
Always double-check units and reference ranges โ these are not automatically applied
Consider adding a free-text comment if values are approximate or transcribed from paper
