What do the business rules mean?
What makes a patient eligible?
Patients are eligible for the DM021 denominator if they:
- are on the Diabetes register
- are living with moderate or severe frailty
How to achieve the indicator
To meet the criteria for DM021, eligible patients must have had:
- an HbA1c in the contract year that is less than or equal to 75
What will remove patients from eligibility?
- Patients who do not have an HbA1c recorded during this contract year, but they do have a serum fructosamine result recorded
- Patients who are coded for maximal tolerated diabetes treatment in the contract year
- Patients for whom diabetes quality indicator care was unsuitable in the contract year
- Patients who chose not to receive a blood test in the contract year
- Patients who chose not to receive diabetes quality indicator care in the contract year
- Patient who have been invited twice (at least 7 days apart) for review with no subsequent HbA1c result
- Patients whose most recent HbA1c result in the contract year was followed by two invites at least 7 days apart (ie: a 6 month review or a raised result which needs re-checking)
- Patients who were diagnosed with diabetes in the last 9 months of the contract year
- Patients who were newly registered with the practice in the last 9 months of the contract year

If a patient achieves the
indicator at any point during the year, any exclusions that have also been
added are ignored.
