This template supports delivery of the Investment and Impact Fund (IIF) indicators related to annual health checks for people with learning disabilities. Specifically, it supports:

  • Completion of a Learning Disabilities (LD) health assessment

  • Documentation of a Learning Disabilities health action plan

  • Recording of the patient’s ethnicity

Practices are incentivised through IIF to complete these reviews, making correct recording essential for contract achievement.


πŸ“Š High-level overview of the specification requirements

To meet the IIF indicator for patients on the LD register, the following must occur within the same contract year:

  1. A Learning Disabilities health check is completed

  2. A Learning Disabilities health action plan is coded on or after the date of the health check

  3. Ethnicity is recorded, or a valid exemption is documented

If the patient declines either the health check or the action plan, this must be coded appropriately to avoid performance failure.

  • Tick if patient has declined the annual LD health assessment

These codes will exception report the patient for IIF reporting.

  • Dropdown to record ethnicity

  • Options to tick:

    • Ethnic category not stated

    • Patient refused to give ethnicity

At least one of these three options is required to satisfy the IIF criteria.

  • Action plans coded before the health check do not count – ensure the code date is correct.

  • You can use a separate template for the full health check, but this screen ensures IIF compliance is clearly documented.

  • If the patient lacks capacity or declines participation, record the appropriate decline or unsuitability code.

  • Ethnicity recording can come from any valid source β€” consider pre-populating from existing records if possible.

  • Encourage use of the narrative field to summarise care and support planning β€” this is helpful for continuity of care and audits.
