Suggested activities to undertake early in the contract year to aid QOF performance and organisation

Ensure that time sensitive QOF indicators are followed up

Where information can be collected via questionnaire to complete QOF indicators

Review high risk patients who need following up

Review patients who have consistently not engaged with monitoring

Review those with PCAs in previous years to see if these need to be applied again

Where patients are in care homes/homeless or have other characteristics where you may want to approach them as a group for all conditions, these searches aid this approach

Identify where patients need following up to help achieve the more stringent QOF targets.
