This template brings together all diary recalls into one workspace, covering long-term conditions, diagnostics, follow-ups, medicines, women’s health, mental health and routine observations. It includes an Introduction page showing the patient’s current due/overdue recalls, and pages for creating new recalls.
🧠Quick vs Full Pages – What’s the Difference?
Quick (Q) pages: date only — you select the recall date from a picker.
Full pages: date + notes — you can add a short text note alongside the recall date for extra context.
The set of conditions/tests on each Q page is the same as its Full page counterpart; the only difference is whether you can add free text.
📋 What’s on Each Page
Shows the patient’s diary entries that are overdue or due within the next 28 days.
Support text with links for reporting issues or requesting developments, plus a contact number.
Acts as a dashboard: you can see, for the loaded patient, which reviews/tests need action now.
Covers QOF-driven LTC reviews, including:

Coronary heart disease annual review and monitoring
