This template brings together all diary recalls into one workspace, covering long-term conditions, diagnostics, follow-ups, medicines, women’s health, mental health and routine observations. It includes an Introduction page showing the patient’s current due/overdue recalls, and pages for creating new recalls.


🧭 Quick vs Full Pages – What’s the Difference?

  • Quick (Q) pages: date only — you select the recall date from a picker.

  • Full pages: date + notes — you can add a short text note alongside the recall date for extra context.
    The set of conditions/tests on each Q page is the same as its Full page counterpart; the only difference is whether you can add free text.


📋 What’s on Each Page

Introduction

  • Shows the patient’s diary entries that are overdue or due within the next 28 days.

  • Support text with links for reporting issues or requesting developments, plus a contact number.

  • Acts as a dashboard: you can see, for the loaded patient, which reviews/tests need action now.


Long-Term Conditions – National Contracts (QOF)

Covers QOF-driven LTC reviews, including:

  • Asthma annual review and follow-up

  • COPD annual review and follow-up

  • Coronary heart disease annual review and monitoring

  • Heart failure annual review and follow-up

  • Hypertension annual review and monitoring

  • Diabetes annual review, 6-month review and follow-up assessments

  • Atrial fibrillation annual review

  • CVA/TIA annual review

  • Dementia care plan review

  • Learning disabilities annual health check

  • Rheumatoid arthritis annual review

  • Chronic kidney disease annual review and monitoring

  • Peripheral vascular disease annual review and monitoring

  • Others included in the QOF LTC contract set


Long-Term Conditions – Other (Non-QOF)

Covers non-contracted LTCs, for example:

  • At risk of diabetes mellitus

  • Cancer care review

  • Coeliac disease annual review

  • Osteoporosis monitoring and follow-up

  • Epilepsy monitoring

  • Rheumatology follow-up

  • Dermatology follow-up appointments

  • Inclisiran recall

  • Other practice-defined long-term conditions not part of QOF


Diagnostics

Covers diagnostic recalls such as:

  • Chest X-ray (standard or plain)

  • Ultrasound scans (including echocardiogram/heart scan)

  • Colonoscopy (including “check colonoscopy”)

  • DEXA bone scan

  • Audiogram

  • Mammography

  • FIB-4 score and NAFLD score

  • Liver elastography

  • Biopsy administration



Generic Follow-Up

For setting non-specific recalls:

  • Follow-up encounter

  • Generic follow-up



Mental Health

For mental health follow-ups:

  • Depression interim review

  • Mental health review follow-up

  • ADHD annual review

  • Mild cognitive impairment review



Women’s Health & Contraception

For recalls relating to contraception and women’s health:

  • Cervical smear

  • Hormone replacement therapy review

  • Oral contraception

  • Depot contraception

  • Subcutaneous contraceptive change

  • Removal/replacement of IUD or IUS

  • Pessary replacement



Injections & Immunisations

For routine injection and vaccination recalls:

  • Vitamin B12 injection

  • Denosumab injection

  • Gonadorelin analogue (implant, SC or IM injection)

  • Vitamin D injection

  • Hepatitis A vaccination

  • Pneumococcal vaccination

  • Herpes zoster vaccination

  • General IM or SC injections


Observations or Admin

For routine nurse/HCA tasks:

  • Blood pressure readings (routine or raised)

  • BMI

  • Pulse

  • Spirometry

  • ECG

  • Doppler ultrasound

  • Wound care

  • Urinary catheter attention

  • NHS Health Check follow-up


Medicines Actions

For recalls relating to prescribing and medicines safety:

  • Medication review (various types — pharmacist, community pharmacist, extended, optimisation, reconciliation)

  • Restart of medication

  • Synchronisation of repeat medication

  • Opioid or other dosage tapering

  • Controlled substance checks

  • Pharmacological assessment

  • High-risk drug monitoring


Blood Tests

For recalls relating to pathology tests:

  • Full blood count (FBC)

  • HbA1c / diabetic control

  • Lipid profile

  • Serum calcium, magnesium, phosphate, ferritin, urate

  • Folate and vitamin B12

  • Vitamin D

  • Renal function, liver function, PSA, thyroid tests

  • CRP, ESR

  • Tumour markers (CA125, CEA)

  • Urine dipstick, ACR, culture

  • Hormone assays (oestradiol, testosterone, prolactin, parathyroid hormone)

  • Others as configured
