Getting started with Questionnaires
Resources → Questionnaires is where you build and manage patient-facing questionnaires — with pages, branching and a proper publish lifecycle so a live questionnaire never changes under your feet.
The questionnaire list
- Filter by status — draft, published, archived or retired — and search by name.
- New Questionnaire opens the questionnaire builder.
- Import brings in an existing questionnaire definition instead of starting from scratch.
- Duplicate an existing questionnaire to use it as a starting point.
Building a questionnaire
In the builder you add pages and questions — multiple choice, scales, free text and coded answers (answers linked to clinical codes so responses can land meaningfully in the record).
The lifecycle
Questionnaires follow a simple lifecycle:
- Draft — work in progress; edit freely.
- Published — ready for use. Published versions stay stable while you draft changes, so what patients see doesn't shift mid-flight.
- Archived / Retired — superseded questionnaires are kept for reference rather than deleted.
