Care Plan launcher (HP155)

Purpose:

This protocol will offer a care plan to the user at the point of saving a consultation if they have coded a review during the consultation

What does it actually do?

The protocol offers a care plan or information provision is needed for QOF, IIF or nationally enhanced services.  This includes:
  1. Asthma
  2. Cancer
  3. COPD
  4. Dementia
  5. Diabetes
  6. Learning disabilities
  7. Mental health
  8. Personalised care and support plan

What does it look like?

Depending on the review undertaken, the offer will appear accordingly:

This protocol is an optional handy protocol.  You can request if via the PCIT hub or submit a ticket to request installation.

Primary Care IT are pleased to announce the release of our Care Plan Launcher.  This Handy Protocol will automatically offer the creation of a care plan where a review has been recorded during a consultation.  This offer is created at the point of saving the consultation.  This is an optional handy protocol which can be requested via the PCIT hub or submit a ticket to request installation.
