Signing your organisation up to Nexus

Before you start — do you need this? This wizard is for buying the paid platform, and only one authorised person per practice does it. If you just want the benchmarking dashboards, they're free for all NHS staff — no registration of your organisation needed. If your practice already subscribes, join its existing account instead. Who should register first? The first person to register a practice becomes its Nexus administrator and controls who else at the practice can access it — so this first registration should be made by the practice manager or a senior partner. We check this when reviewing each request; if someone else registers first, we'll email them asking their practice manager to register instead. Once the administrator is set up, they invite everyone else from within Nexus in a couple of clicks.

Registering your GP surgery on Nexus is a three-step wizard. You'll need your NHS email address and, ideally, your surgery's ODS code. It takes about five minutes; approval then typically takes 1–2 working days.

From the signed-out home page choose Medicus customer → Register your GP surgery (or follow a registration link you've been given).

Step 1 — Your details

Enter your full name and work email. The email must be an NHS address (@nhs.net or an nhs.uk address).

Step 2 — Find your surgery

First tell us what you're registering — a GP practice, a PCN or an ICB. Practices continue below; PCN and ICB registrations are covered in Registering a PCN or ICB.

Search for your surgery by name or ODS code — the search runs against the live NHS Organisation Data Service. Select your practice from the results; you'll see its name, ODS code, address and status to confirm it's the right one.

If your surgery doesn't appear, ask your IT team for the practice's ODS code.

If your practice is already on Nexus, the wizard tells you so and switches to a Request access flow instead — your practice's existing Nexus administrator is asked to approve you, and there's no billing or setup to do. See Joining your organisation's existing account.

Step 3 — Patients, plan and payment

  • Registered patient count — filled in automatically from the latest NHS publication where available; otherwise enter it yourself. (You can update it later in Practice → Settings.)
  • Clinical system — EMIS Web, SystmOne (TPP), Medicus, Vision (Cegedim) or Other. This tailors your onboarding and which build tools you're offered.
  • Medicus practice ID — *shown only if you chose Medicus, and required.* The six characters in the address bar of your own Medicus instance, right after medicus.health — in https://england.medicus.health/cd4c58/scheduling/homepage that is cd4c58. You can paste the whole address and we'll pick it out. It is not your ODS code. See Medicus practices — getting your data into Nexus.
  • Your price — there is no plan to choose. From the patient count above, the wizard shows your population band and what the Nexus Core Platform costs at that band as an annual licence excluding VAT. Solution modules are added separately afterwards. We confirm the band against NHS list-size data when we approve your account, and it is then fixed until renewal. See How Nexus is licensed.
  • How would you like to start?
  • Free 90-day trial — no card details needed; billing is set up when the trial ends.
  • Subscribe now — card details are collected at the end via Stripe. Your card is validated and saved, but nothing is charged until your account is approved.

*Medicus practices don't see this choice.* A three-month free trial comes with the Medicus route automatically, no card is asked for, and the three months start from the day your data is live rather than the day you register — see Medicus practices — getting your data into Nexus.

  • Trial code (optional) — if Primary Care IT has given you a trial code, enter it here. A valid code applies its own trial length and skips payment entirely. See Trial codes.
  • Administrator confirmation — confirm whether you're authorised to act as the administrator for the organisation. The first user of a new organisation becomes its admin on approval.

An estimated monthly cost is shown throughout, based on your patient count and chosen tier. All prices exclude VAT.

Registering a PCN or ICB

Primary Care Networks and Integrated Care Boards can register their own Nexus account. Choose PCN or ICB at the top of the *Find* step, then pick your organisation from the NHS directory (it's searched by name; every PCN and ICB in England is listed).

A PCN or ICB registration is shorter than a practice one:

  • No patient count, clinical system or plan step — those only apply to practices. Your registration is submitted straight after you pick your organisation.
  • No card details — nothing is charged at signup. Packages and pricing for network- and system-level organisations are agreed with the Primary Care IT team after approval.
  • The first person to register becomes the organisation's administrator, the same as for a practice, and invites colleagues once approved.

Approval works the same way: our team verifies your role, and you'll receive a password-set email — typically within 1–2 working days.

What happens next

After you submit (and, for paid sign-ups, confirm your card with Stripe):

  1. The Primary Care IT team verifies your membership of the surgery.
  2. Once approved — usually within 1–2 working days — you receive an email with a link to set your password and sign in.
  3. Billing starts on approval, or after your free trial if you used one.

If you cancelled out of the Stripe page by mistake, the confirmation screen offers an Add payment details button to retry — or you can register again and choose the free trial.
